Lone Star Family Health Center Awarded $35,750 from Health and Human Services for Being a Quality Leader in Healthcare U.S. Department of Health and Human Services Secretary Susan Burwell announced Lone Star Family Health Center will be awarded $35,750 in recognition of quality health center improvement achievements. “This funding rewards health centers that have a proven track record in clinical quality improvement, which translates to better patient care, and allows them to expand and improve their systems and infrastructure to bring the highest quality primary care services to the communities they serve,” said Secretary Burwell. “With these funds, health centers will continue to provide access to high quality, comprehensive primary and preventive health care to the patients that need it the most.” “We are 1 of about 1,000 health centers nation-wide to be rewarded financially for being proven leaders in areas such as chronic disease management, preventive care and for the use of Electronic Health Records (EHRs) in reporting quality data,” said Dr. Stephen McKernan, Lone Star Family Health Center’s CEO. Lone Star Family Health Center (LSFHC) receives this financial distinction for two reasons. First, the EHR system at the clinic captures and reports all clinical quality measure data for patients. Second, in the past year, Lone Star’s patients have demonstrated more than a 10% improvement on many clinical measures. Increased colorectal and breast cancer screening rates and a decrease in the percent of babies born at a low-birth weight are among some of the specific measures for which LSFHC is recognized and rewarded. “These funds reward and support those health centers that have taken steps to achieve the highest levels of clinical quality performance and improvement,” said Health and Resources Administration (HRSA) Administrator Mary K. Wakefield, Ph.D., R.N. Founded in 2002, Lone Star Family Health Center is a Federally Qualified Health Center with a mission of providing high quality, easily accessible health services to the underserved and the community at large providing comprehensive primary care services for all ages. The health center’s main location is at 605 South Conroe Medical Drive next to Conroe Regional Medical Center with a satellite clinic in Spring, TX, at 440 Rayford Road, Suite 150. Appointments may be scheduled by calling 936.539.4004.
