This is a good overview of the present state of our guidelines, and discusses some of the problems with the present two-tiered testing, and how chronic Lyme disease impairs the quality of life of affected patients. Because there are two standards of care in Lyme disease, patients should be advised of the benefits and risks associated with both treatment approaches and engage in shared medical decision making with their physicians that recognizes the importance of patient values and preferences in decisions that impact quality of life. Shared decision making is a collaborative process by which patients and their providers make healthcare decisions together, taking into account the best scientific evidence available, clinical judgment, and the patient’s values, preferences, and circumstances.
