Need some advice. One crew (emt and emr) is on call 24/7 covering a large geographic area. Nearest hospital is 45 minutes away when weather conditions are favorable and wildlife stays in the fields. When toned, we have second car staffed with 2 emrs. On occasion, we are asked to do transfers for other understaffed ems services. We typically turn down the offer if it leaves our area with no EMT coverage. However, when one of our community residents requires transport from hospital to hospital we typically accept this transfer with the understanding that our ems service is there for our community and their families. This leaves our service area without EMT coverage for hours (can be up to 12 hours before we are back to our service area). We do have emr coverage but our emrs are increasingly becoming uncomfortable with this responsibility on their shoulders. What do you think? Turn down all transfers in order to maintain EMT coverage for the area? Thank you for your perspectives...
