Full Time Employment Job Summary: The Homeless Prevention Program (HPP) provides homeless men, women and families support, opportunity and access to services and housing in order to reduce the likelihood of continued homelessness. Services are client-driven and promote long term housing stability. Objectives are to provide assistance and support which break the cycle of homelessness and reduce the time that individuals remain homeless. HPP enhances access to community based support services which help individuals maintain their housing and connects homeless individuals to income assistance where needed. HPP also promotes housing stability by providing supports for both landlords and tenants. Reports To: Executive Director and/or Program Manager EDUCATION & QUALIFICATIONS: Minimum grade 12 with appropriate secondary education such as: • Social Service Worker certificate and/or related certificates in human service field • Must have computer knowledge and ability to use Microsoft Word & Excel programs in a proficient business manner • Previous successful experience running a social service program • Exceptional oral and written communication skills • Ability to integrate and contribute to a well established team • Excellent record of attendance & time keeping as proven by past history • Must have own vehicle with valid driver’s license and clean drivers abstract RESPONSIBILITIES The Homeless Prevention Program Outreach Worker provides the following services: a) Engage with unsheltered homeless individuals; and provide supports which will help individuals stabilize their lives b) Connect individuals with income support, including making and accompanying individuals to appointments; c) Where possible assist individuals to locate a rental unit and ensure that an intent to rent form and/or social housing application have been completed; d) Provide follow-up supports to ensure individuals have effectively transitioned to housing and receive related health and social service programs and services; e) Provide, or provide links, to skills training in life skills, personal health and hygiene, household management, financial management, crisis intervention and problem solving, and follow up related to these areas. f) Build and maintain relationships with landlords who provide housing to clients. Includes providing landlords with resources and education to support and maintain client tenancies; g) Disburse rental supplements to eligible clients; h) Complete all file keeping, statistical and database requirements; i) Connect and engage in partnership with relevant community services. j) Relevant case planning for each individual client, referrals to appropriate support services and documentation GENERAL RESPONSIBILITES a) Attend and participate in all staff and program meetings; b) Actively cooperate with other staff and the Executive Director/Program Manager in decision making regarding program operations; c) Bring major issues, needs, and concerns to the attention of the Executive Director/Program Manager; d) Adhere to the Society’s Policies and Procedures and Mission Statement; e) Review the policies and procedures regularly and obtain clarification from the Executive Director/Program Manager on any problem areas or concerns; and, f) Treat as confidential the business of the clients and the Society as a whole.
