New guidelines from the International Society of Geriatric Oncology suggest that age alone should not be used to guide decisions about treatment for prostate cancer in older men. The guidelines advise that older men be classified as fit, vulnerable, or frail, based on factors including health, independence in activity, and nutritional status. Fit older men should be treated for localized or advanced prostate cancer the same as younger men, vulnerable men should have their underlying problems addressed when possible, and treatment for frail patients should focus on symptom relief and palliative care.
