Whether there are benefits of these therapies in other categories of pulmonary hypertension is unclear. With the lack of randomized clinical trials of approved therapies in other categories of pulmonary hypertension, it may be improper to treat patients with pulmonary hypertension, other than those with PAH, with current approved therapies.[47] Second, cost of therapy is of concern as PAH-specific therapies are expensive. It is estimated that annual cost for sildenafil, bosentan, ambrisentan, and iloprost is $12,761, $55,890, $56,736, and $92,146, respectively.[3] As epoprostenol and treprostinil are dosed by weight, based on a 70-kg patient on the lower end of the dosage range, the annual cost is approximately $33,153 for epoprostenol and $97,615 for treprostinil.[3]
