See the list below and check off all, if any, that apply. □□ Are they waking up and getting dressed at a reasonable time every morning? □□ Are they wearing clean clothes every day? □□ Are they doing their hair every day? □□ Is he clean shaven? □□ Is she wearing makeup? □□ Do they call important people in their life by their correct names? □□ Is their speech any different? □□ Are they up to date on the news? □□ Have they continued in their regular activities and hobbies? □□ Do they repeat things? □□ Do they get fixated on certain things? □□ Does their living environment have an unpleasant odor? □□ Is the kitchen clean? □□ Is there old food in the refrigerator or cabinets? □□ Has the garbage been taken out? □□ Is there an unusual amount of clutter? (i.e. mail, laundry, dust, dirt.) □□ Do they appear to be taking their medications properly? □□ Do they appear to be moving around safely? □□ Do they appear physically healthy?
