One thing that weve learned from Pit Crew CPR is that while guidelines and protocols tell you what to do they do not tell you how to do it and thats where the real action is when it comes to process improvement. When I review PCRs Im not a fan of the phrase ACLS guidelines followed throughout. Its a meaningless statement that conveys nothing useful. Beyond that, its almost certainly not true. A mediocre attorney could poke holes in it with minimal effort. Id much prefer initiated resuscitative efforts using the Pit Crew CPR process and the checklist. Thats something we can back up because its evidence-based and its true. The chain-of-survival is a useful concept for cardiac arrest survival at the community level but its not something that helps ensure best practices are implemented on emergency scenes. For that you need a different paradigm that focuses on decision making, teamwork, and system performance. This is one of many areas where our current educational model has really let us down, at least in the United States!
