PATIENT CARE COORDINATOR - Full-Time position at Children’s Specialized Hospital, Mountainside. Job Description: functions in team-based, collaborative setting to coordinate care of patients in a family centered manner. An active liaison between patients / families and multidisciplinary team including medical subspecialists, therapies, family resources, educational services, insurance companies and external agencies. Helps patients/families shape and reach goals so that patients may meet their full potential in a medical home based model. Provides care coordination consistent with a “Best of Breed” medical home. Essential Job Functions: Communicates in an exemplary fashion; Effectively engages patients/families in helping relationships using acquired professional skills; Demonstrates an understanding of the patient’s medical diagnoses, prognosis, progress and implications for family support, education and goal directed treatments and therapies; Demonstrates an understanding of the health care system, managed care and resource utilization. Advocates for the unmet needs of patients/families across the medical, social and financial continuums; Completes proper documentation in a timely manner; Provides necessary documentation to external agencies as needed; Participates actively in projects that are consistent with department and hospital goals including those relevant to medical home certification. Possesses goal/outcome orientation; Desires continuous learning. Education and Experience: Bachelor’s degree in Nursing, Social Work or related Field. Master’s degree preferred.
