Medicaid expanded to include low income households not just poverty level. All households pay “deductibles” based on sliding scale determined by income. Physician Assistant or Nurse Practitioner run “care centers” located within walking distance of city hospital emergency rooms. ER patients are triaged in waiting rooms for level of care and sent to these less expensive care providers if appropriate. Hospitals could help fund with savings from treating uninsured with non -emergencies.
