Patient Care Facilitator FACILITY LOCATION: Webster City, IA Brighter Day Health Brighter Day Health (BDH) is a national provider of psychiatric and psychological care to residents in skilled nursing facilities. The company incorporates the latest technologies to deliver the highest quality health care via telemedicine. BDH is seeking a Patient Care Facilitator who will physically be on-site in a Skilled Nursing Facility (SNF). As a Patient Care Facilitator, you will have the following general responsibilities: • Travel to the facility on your assigned days. • Ensure that the residents attend their appointments on a timely basis. • Obtain current copies of each resident’s records or other information and fax them to BDH. • At all times, conduct yourself in a polite, professional manner; stay focused at the task at hand; and safeguard all information protected under HIPAA. You will be required to pass a background check and a drug test. You must have reliable transportation. CNA or healthcare background preferred.
