Plenary Session - Tackling Adherence and Retention The first speaker, Pierre Barker is speaking on Fixing the Cascade. Barker is emphasizing the point that the inflow or new ART initiates have to be balanced by people leaving the system of treatment for there to be a steady state that the country can support. The rate of entry and exit become key to understanding the success or failure of local treatment programmes. Thus by changing the CD4 threshold we are drastically altering the pool of people that need to be treated. Meaning for each district its useful to calculate the backlog and HIV population in need of treatment in order to strategically plan treatment roll out. He elaborated on the breakthrough system and adherence clubs as a useful model for scale-up, as they use network learning sessions and feedback sessions to move programmes forward. Ideas that emerge from these learning sessions that are successful can be quickly implemented in other districts/clinics. These models can also be applied to Chronic Care areas.
