Position Details Reference Number 160895 Position Title CNC Chronic & Complex Care Employment Status Permanent Full Time, Permanent Part Time Entity Far West Local Health District Geographical Location Broken Hill Award Classification Clinical Nurse Consultant Salary $48.04 - $49.02 Hours per Week 38 Number of FTE 1.00 Purpose of Position The Clinical Nurse Consultant in Chronic Disease –Particularly those diagnostic groups related to severe chronic disease ( Heart Failure, COPD, Hypertension, Diabetes and Coronary Artery Disease, is qualified to make independent and / or collaborative decisions in partnership with individuals, families or communities. Their practice will involve providing clinical services and consultancy, clinical leadership, research, education and participation in clinical services planning and management. Clinical Nurse Consultant Chronic Disease – •Be an integral member of the Chronic and Complex Care Program, provide care coordination and referral across to General Practitioners •Promote and support the principles of chronic disease self management and conduct education sessions and programs to support community development, health promotion, early identification and prevention •Prioritise and implement health promotion initiatives according to strategic direction Far West Local Health District farwesthealthjobs.au , NSW Health •Provide expert clinical advice to patients, carers and other health professionals within the management of chronic diseases •Develop, facilitate the implementation and evaluates care management plans for patients with complex health needs with •Provide leadership that facilitates the ongoing development of chronic disease clinical practice and service delivery for •Initiate and utilise the findings of research in the provision of clinical services, clinical guidelines/pathways and audits for •Contribute to the development and delivery of Chronic Disease - and / or related education programs •Participate in formal processes for the strategic and operational planning of the service. This role also involves the organisation and delivery of Chronic Disease services •Participate in multidisciplinary team meetings and collaborate with other health professionals as needed •Refer to other health professionals as necessary •Accept referrals from other health professionals •Coordination of Outpatient Cardiopulmonary Rehabilitation Program and collaboration with others in the development and delivery of education programs •Delivery and coordination of Outpatient Chronic Care Nurse Clinic through the Outpatient Department •Coordinate and deliver disease specific community health promotion. Facilitate, evaluate and report on all health promotion programs. For more information please see immi.gov.au Selection Criteria Currently registered as a Registered Nurse with the Australian Health Practitioner Regulation Agency. At least 5 years full time experience as a registered nurse. Post Graduate qualifications relevant to Chronic and Complex Diseases with at least 3 years full time equivalent experience in that field within the last (8) years. Demonstrated clinical expertise and clinical leadership abilities in Chronic and Complex Diseases Management Demonstrated time management and organisational skills with excellent written and verbal communication skills Demonstrated ability to work collaboratively in a multidisciplinary team. Experience in developing and presenting education, clinical procedures and policies to staff and the community Current Drivers License and a willingness to travel around the District This position requires a Working With Children Check (WWCC) issued by the Office of the Childrens Guardian.
