Question from one of our readers: I just began my 2nd chapter in my career as an RDH. Now I am a licensed RDHAP and have begun my exciting new journey. I was recently asked to submit to my patient after completing my service to give them a super bill so they can submit their own insurance claim. Do you know of a form for such super bill? I may need to have a bulk of these printed up in at least duplicate form. What should it contain beside my company name etc. the service and insurance code associated with my service and payment made. Could this all be easily done with a square swipe that would at least give the name of my company etc. on it? I am looking for streamline if such exists. Please leave your feedback/suggestions below.
