REMINDER FROM DSL: September 25, 2013 TO : DSL Artists, Journalists and Selected International Students THRU : Virgilio Castillo Jr. – Head, Office of Culture and Arts Dianne May Torres – Head, Student Publications Office Leandro Loyola – Head, Student Involvement Office (signed) FROM : Karen D. Asuncion Academic Assistant, Department of Student Life RE : Midterm Grade Advising Guidelines The Department of Student Life (DSL) is committed in providing support and assistance with the academic growth and development of our students. In order to carry out this mission, DSL is requiring each and every student to conform with the Midterm Grade Advising every term. The Midterm Grade Advising is a process wherein students need to accomplish the Midterm Grade Form by coordinating with their respective instructors and to submit the accomplished form on or before the deadline to the Academic Assistant. With this, please find below the detailed guidelines, for your reference: 1. Secure a Midterm Grade Form from your respective units or from the Department of Student Life on or before the Midterm Week Examination 2. Coordinate and get from your respective instructors your midterm grade or your midterm grade standing (in absence of the midterm grade) and your areas of improvements should you have “R” grade, passing mark or a grade below 2.0. 3. Accomplished midterm grade form should be submitted on the deadline set by the Department of Student Life which is usually a week after the Midterm Grades due at the Office of the Registrar. (Please refer to the Calendar of Activities released by the Registrar’s Office for the exact date.) 4. While submitting the accomplished midterm grade form, the Academic Assistant of the Department of Student Life shall be evaluating the grades specified on the form and shall be giving advice should there be a need specifically for subjects that have failing grades and other concerns aired up by the student. 5. Students who have difficulty with their subjects shall be endorsed to the Student Learning Center for tutorial services in preparation for the final examination week or to other offices pertaining to concerns not related to grades. More so, DSL shall strictly monitor the compliance of the DSL artists, journalists and selected international students in the requirements set for the midterm grade advising. Students who will not able to do the midterm grade advising may not be able to enlist during the pre-enrollment on the schedules set by the Registrar’s Office effective Second Term, SY 2013-2014. Should there be other concerns or queries regarding the matter, you may visit us at the 3rd Floor, MFC building, College of Saint Benilde Taft or reach us at telephone number 5267441 local 250. Please be guided accordingly. Thank you very much. Noted by: (signed) Norie M. Ador Dionisio Dean, Department of Student Life
