REMINDER: Those students not presenting evidence of having received the booster dose by August... 29th, 2014 will be excluded from school and not allowed to return until the record is received. Ohio Law says that: • Students in grade 7-8: Require one (I) dose of Tdap (Td is not acceptable). • Any student who has received a dose of T dap at any time will meet this requirement and not need to be re-immunized for entry into 7th -10th grade. Please contact your childs doctor/clinic to schedule an appointment. The Butler County Health Department provides the vaccine for a small charge by appointment only. You may reach the Health Department at 513-863-1770 for additional information and their immunization clinic schedule.
