Report on the incident debrief held at Eagle Park on 6 January 2015 SSAA (Vic) facilitated a debrief on the incident where, Pyle Reid, a 22 year old was injured after being struck by an unidentified projectile. Police, Worksafe Victoria, Range Officers and State Office staff attended. No clear conclusion was reached as to how an unidentified projectile hit Mr Reid. The general consensus was it was a freak incident. As a result Police, Worksafe and SSAA deemed the range safe and it re-opened the day after the incident. Much of the debrief therefore focused on the post incident response. The objectives were to discuss what went well, what could be improved and to identify any conclusions and outcomes. All parties involved in the incident and post incident investigation had an opportunity to present at the debrief. Summary of key outcomes • Eagle Park staff acted in a professional manner in response to the incident • Incident scenes, such as this, need to be secured until Police/Worksafe have completed their investigations. • The section on emergency procedures in the ‘range standing orders and procedures’ requires review • The range should record the contact details of all shooters on entry to the range • The standards for the placement of swinging targets on the main public range need to be reviewed
