SOUTH CENTRAL AMBULANCE SERVICE NHS FOUNDATION TRUST advises: We would like to invite you and your members and/or colleagues to attend our first Buckinghamshire constituency meeting and Patient Forum on Tuesday 3 September from 7pm to 9pm at St Anne’s Hall, Wendover. These meetings are held in public and members of the public, as well as members of the Trust, are welcome to attend. The above meeting will also give you the opportunity to join our Patient Forum. SCAS will launch this new initiative at the Wendover constituency meeting and it is keen to get as many members and non-members involved. These patient forums will provide a stronger link between the trust and their members and improve communication by keeping local people informed of any changes and gathering feedback on existing services. Their key role will be to: · Enable members and non-members, in particular those who are (or have been) patients to have a say on how the trust operates · Support the trust in providing a high quality of care and service delivery by helping with various activities such as conducting patient surveys and offering views on proposed new services. Terms of Reference and Honorary contract and Nomination form for Patient Forum) 19.20– 19.40 Presentation - You and your ambulance and the role of your governor 19.40– 19.55 Presentation - Local health issues, from an ambulance perspective 19.55 - 20.15 Groups discussions 20.15– 20.35 Feedback from groups 20.35 – 20.40 Collection of Patient Forum signed documents 20.40– 20.50 Your views on this forum/any other business 20.50 – 20.55 Announcement of nominated vice chair. What happens next 21.00 Close Your local ambulance trust is a vital part of the local health network and I hope you will be able to make it on September 3rd.
