Shoulder pain is the second to third most common musculoskeletal complaint in the general population. Studies have found that up to 60% of people will experience shoulder pain at some point in their life and, importantly the incidence increases with age. Furthermore, shoulder pain can been very debilitating and have a serious impact on sufferers. Shoulder pathology is associated with a high morbidity rate with reliable studies suggesting that whilst 50% resolve within 6 months, 40-54% of people report on-going symptoms for 1-3 years. Recent advances in MRI and Ultrasound have allowed us to peer into the shoulder in ever greater detail and the results have been startling. The simple fact is that our shoulder joints were not designed to last more than about 40 years; we have built-in obsolescence! Soft tissue pathologies for the rotator cuff, for example, are ubiquitous, with 96% of ‘asymptomatic’ men over 70 demonstrating a moderate to severe shoulder cuff pathology.
