Silicosis with progressive massive fibrosis. A, Chest radiograph shows bilateral upper lobe masses. The left upper lobe mass is relatively peripheral, and associated with marked volume loss with leftward herniation of the trachea and right lung. The more central right upper lobe mass is partially obscured by the overlying sternum. There is moderate background nodularity. Pleural thickening and calcification are evident on the left. B, CT shows a large mass in the medial right upper lobe, with a smaller mass on the left. There is centrilobular emphysema, and bilateral peripheral cicatricial emphysema. Scattered small nodules are predominantly subpleural. Subpleural clustering of nodules on the left has created a pseudoplaque (arrows). C, CT through the lower lobe shows typical appearances of rounded atelectasis, related to an area of pleural thickening.
