States make a big difference in the implementation of federal programs. This review of the responses in VA and MD is eye-opening. MD has 325 trained workers to help its 800,000 uninsured navigate health marketplaces. VA has about 24 guides to assist 844,000 uninsured. MD set up its own exchange and got $157 million in federal grants. VA decided the federal option was preferable to creating its own and will get just $6 million.
