Stories from my Practice Doctor I feel so tired, Can you help me ! When Mr JKU walked in my office there was nothing unique about his complaint 51 year old male, obese, busy as a marketing executive. He had not had a checkup for several years. Recently he started feeling tired. No energy. He still continued to push himself, do his job and perform his regular duties. His primary care doctor did all the initial workup that included blood work, X ray and an EKG, yet his EKG was abnormal showed left bundle branch block. That alarmed the doctor and he immediately referred the patient to me. There was nothing alarming in his history and physical he had high blood pressure, high cholesterol otherwise no thing. He had cold symptoms few month ago and he treated that with cold medications. Our initial work up included an Echo cardiogram ( ultrasound exam of the heart) that showed that the heart muscle is severely damaged working only at 15 percent of the its capacity. After absorbing the initial shock, me and him went back and reviewed all his medical history for ten years and there was no other incidents. We decided to proceed with heart catheterization ( taking x ray pictures with iodine dye of the coronaries arteries) to evaluate for the blockage Again there was no blockage but dilated and weak heart muscle We started medical therapy to very important medications were used angiotensin Inhibitors and beta blockers to help the heart heal and function better. A strict diet and exercise program was followed. Symptoms of fatigue and shortness of breath gradually improved, pt life style went back to normal and his functional status. The heart function stayed very low but without an effect on patient lifestyle or carrier. What is Idiopathic Dilated Cardiomyopathy
