 Summitville Country Fair HOG WRESTLING CONTEST FRIDAY- JUNE 28, 2013 Entry Fee: $40.00 per Team Registration Sign-up Starts @ 6:30 p.m. Event Start: 7:30 p.m. Age Classes: Boys: 9 - 17, 18 & up Girls: 9 -17, 18 & up Mixed Boy & Girls: 9 -17, 18 & up WE STORNGLY ENCOURAGE YOU TO REGISTER BEFORE THE DAY OF THE EVENT BY MAILING OR DROPPING OFF YOUR COMPLETED ENTRY FORM AND FEE TO THE SUMMITVILLE MAIN ST. ORG., 149 E. 1450 N. SUMMITVILLE, IN 46070 Participants under 18 years old MUST HAVE a parent sign a release form. RULES / INFORMATION Pre-registration participants must have all participant forms signed and money paid at pre-registration. Only one (1) team member at the sign-up table needed to receive and hand in signed papers and money. Unless under 18, then one (1) team member and one (1) parent. Please dress for mud and proper shoes MUST be worn or competitors will be disqualified. Wet clothes WILL NOT be permitted in any other area of the fairgrounds other then designated area. Wash station will be provided for clean up. BRING CHANGE OF CLOTHES. No mudslinging inside or outside of arena. Participants WILL NOT be allowed in the grandstands or restrooms in the buildings unless participants are cleaned up. Circle ring will be approximately 17 feet. Team must catch and place the hog on barrel with bare hands in 1 minute or less. Fastest time wins. NO artificial catch devices. Unnecessary cruelty will disqualify team. DO NOT put the hogs head under the water/mud. Fair committee will open gate for hog to jump into the arena. When the hog is in the arena, the gate will open and the clock will start. When the hog is placed fully on the barrel the clock will stop. Teams must have the hog in the exit chute after each run or they will be disqualified. Judges (timer, starter and gate person) decisions are final. The fastest team over all shall receive their entry fee back with a winning trophy. HOG WRESTLING CONTEST FRIDAY - JUNE 28, 2013 Participant Agreement, Release, and Acknowledgement of Risk In consideration of the Summitville Main St. Fair Board the agents, owners, officers, volunteers, participants, employees and all other persons or entities acting in capacity in their behalf. I hereby agree to release and discharge Summitville Community Center, on behalf of myself, my children, my parents, my heirs, assigns, personal representatives and estate as follows: I acknowledge that Summitville Main St. Fair Board, entail known and unanticipated risks which could result in physical or emotional injury, paralysis, death, or damage to my self, to property, or to third parties. I understand that such risks simply cannot be eliminated without jeopardizing the essential qualities of the activity. The risks include, among other things: collision with others, the walls, or other fixed objects, falling down; my own equipment failure of the failure of other’s equipment: my own of others negligence; objects or conditions on the surfaces that may cause me to fall; broken bones, sprains, head and back injuries, abrasions and bruises. Furthermore, has difficult jobs to perform. Summitville Main St. Fair Board. They will seek safety but are not in fallible. They might be unaware of a participant’s fitness or abilities. They may give inadequate warnings or instructions and the equipment being used might malfunction. I express agree and promise to accept6 and assume all the risks existing in the activity. My participation in the activity is purely voluntary, and I elect to participate in spite of the risks. I hereby volunteer release, forever discharge, and agree to indemnify and hold harmless Summitville Community Center grounds and all claims, demands, or causes or action, which are in any way connected with my participation in this activity or my use of their equipment of facilities, including such claims which allege negligent acts or omissions of the Summitville Main St. Fair Board. Summitville Main St. Fair Board or anyone acting on their behalf be required to incur attorney’s fees and cost to enforce this agreement, I agree to indemnify and hold them harmless for all such fees and costs. I certify that I have adequate Insurance to cover any injury or damage I may cause or suffer while participant, or else I agree to bear the costs of such injury or damage to myself, I further certify that I have no medical or physical conditions which could interfere with my safety in this activity, or else I am willing to assume and bear the costs of all risks that may be created directly or indirectly by any such condition. In the event that I file a lawsuit against the Summitville Main St. Fair Board, I agree to do so solely in the state of Indiana and I further agree that the substantive law of that state shall apply in this action without regard to the conflict of law rules of that state. I agree that if any portion of this agreement is found to be void or unenforceable the remaining portion shall remain in full force and effect. By signing this document, I agree that if anyone hurt property is damaged during my participation in this activity, I may be found by a court of law to have waived my right to maintain a lawsuit against Summitville Main St. Fair Board on the bases of any claim from which I have released them herein.
