Surgery: Previously treated with an acute apical abscess on #12. Radiographically, the pre-op shows a mid root perforation with an extended Thermafil carrier and associated periapical pathology. Patient factors are mainly economic so the choice for surgery was taken as well as predictability in locating and treating the perforation. Patient was on Aggrenox. The root was resected below the level of the perforation and the main canal was merged with the perforation. The apical retrofilling was IRM. The last radiograph is the 9 month recall, patient is currently asymptomatic.
