The Affordable Care Act for the East Bay is holding its last Healthcare Event this Saturday at the Rockridge Library from noon to 3. Enrollers will assist you in completing your application. This is a free service for our community. Bring: -Your Covered California username,password and PIN number. * If no account yet: -The full name, date of birth and Social Security number of the individuals in your household that you seek to enroll in coverage. -For the primary applicant, ID such as a drivers license or other photo ID. *If not on ID, proof of California residency/home address (e.g. utility bill) *** Household Income information. Most people will find it helpful to have their most recent federal tax return, including any income from dependents. Social Security numbers of non-applicants may be requested, to verify income information. * If foreign born, proof of satisfactory immigration status (Citizenship or Lawful Presence). For example; Legal Residency, Naturalization Certificate, Alien Card, Green Card, etc. YOU MUST FINALIZE BY APRIL 15 TO AVOID PENALTIES
