The Center for Cancer and Blood Disorders JOB POSTING Full-Time, 40 hours/week POSITION POSITION: Patient Navigator DEPARTMENT: Navigator LOCATION: Western Region Contact: Tammy Mask 817-333-0120 tmask@txcc JOB DESCRIPTION: Responsible to serve as a consistent care coordinator throughout the continuum of care assessing the physical, psychological and social needs of the patient. Duties include but are not exclusive to providing educational and emotional support to the patients and their families by being available to answer their questions and address their fears concerning their illness. Facilitates physicians recommendations for patient care to help ensure patient compliance and understanding. Coordinates support groups for patients, their support partners and family. Visits referring physicians on (generally) a weekly basis. REQUIRED SKILLS AND KNOWLEDGE: Clinical background and/or education of healthcare Knowledge of patient education/training practices and theories. Knowledge of formulating education/training goals and objectives consistent with patient needs. Bilingual Preferred Must be able to communicate with patients, peers, physicians, and general public, and be able to handle multiple tasks. Must have general computer skills and ability to learn electronic medical record and computerized scheduling system. Must be a team player
