You pharmacists (whether you work in hospital or not), technicians and assistants. The bad news: people still die unnecessarily in hospitals. Today, The Age reports that someone died last year after being given crushed medicines IV. A tragedy for both the family and the health professionals involved in this sad, preventable, case.
