The good news: advances in medication safety practices mean that fewer people are harmed in hospitals. And thats thanks to YOU. You pharmacists (whether you work in hospital or not), technicians and assistants. The bad news: people still die unnecessarily in hospitals. Today, The Age reports that someone died last year after being given crushed medicines IV. A tragedy for both the family and the health professionals involved in this sad, preventable, case.
