There is intracellular and extra cellular. Intra is just that found inside a cell. Extra cellular is found outside of the cell. Places for Third Spacing Third spaces are in cavities like pleural spaces around lungs and inside of the Abdomen (Peritoneal area) Another type of third spacing is into fleshy tissue areas between fascia and muscle and subcutaneous areas. What can cause the leaking out of extra cellular fluid into these third spaces? Hydrostatic pressure inside of our vascular system is dependent on certain pressures inside of our blood vessels ie veins, arteries and capillaries. I believe hydrostatic pressure is measured in PSI. Capillaries and vessels in our body are porous (little tiny holes), so they are like a garden soaker hose kinda. Suppose you have very little pressure today to put through your hose, thus not much leaking out. So in relation to hose you have very little third spacing Conditions like CHF cause increases in hydrostatic pressure inside of the vessels. Every lb weight gain due to fluid is 2.2 liters of fluid. I had a patient report to me the other day that they gained 30lbs in a weak. That 66 plus pounds increased the hydrostatic pressure in her vessels and fluids that can fit through those tiny pores of our blood vessels leaks into the third spaces in our body and pools into dependent parts of our body. In comparison a garden soaker hose puts out much more water with an abundance of high water pressure. Another cause of third spacing is when people are very sick and septic they have cytokines in their blood stream that cause blood vessels to become leaky. Those pores in blood vessels require less hydrostatic pressure for blood fluids to leak out into the third spaces. Even eye sceleras can becomes fluid filled. Histamine reactions can cause localized third spacing as well. Same mechanisms ...histamine rushes to sight, causes fluid, inflammation and leaky capillaries at the site.
