These are the governments proposed plans for improvements in integrated care for the elderly. It reminds me of a nursing home that I worked in which had three community respite beds paid for by social services. These three beds were never empty and allowed elderly people who were struggling at home to come and have a rest whilst us nurses conducted a specific care plan and identified their individual needs. They were assessed by physiotherapists and occupational therapists during their stay with us and then a proper care package put into place for their discharge either back to their own homes, into hospital or rehabilitation unit or into a long term nursing or residential home placement.
