This Tuesday, they really rushed the doors at closing time tonight. x4 in the clinic at about 6:30 PM one needing major I&D & packing of an enormous MRSA Abscess on her Face. 26 patients total. Very procedure heavy day. Started fast 14 by noon, then backed off until closing. CCP Physical. Gout with tophi in both Great Toes, requiring injection, then oral meds. Acute back pain requiring x-rays and requesting facet injection x2 (one has a party, the other a wedding). Hemorrhoid sclerosing injection, incision, and evacuation of blood clot. Strep. Sinusitis with congestion and cough x4. COPD exacerbation with R Lobar Pneumonia requiring X-ray and nebs. Another COPD with Bronchitis requiring same. Malignant HTN requiring rate and BP control. Severe Neutropenia (1.0), and Leukopenia (0.5), ttl WBCs 1.8, in a 30ish Hispanic mother, with a 4 month old baby, exhibiting fatigue, HA, and Left post auricular adenopathy, a T3 uptake at 20%, Calcium slt. low at 8.6, AST slt high at 53, perinatal HIV & Hep both negative, Normal RBCs - sent to Heme-Onc - mysterious. Epigastritis, arthralgia, and TIAs in 60ish woman with suppurative gingivitis and tooth abscesses. DOT Pe. Influenza type A x2. Motorcycle MVA x3 days needing X-rays of swollen knee and bruised ribs to r/o fractures or pneumothorax. Tinea pedic, cruris, corpus. OM with fever. Mood & Affective DO. I Have had Iain all day, observing. Impressed we hope. Heading home.
