This is just one person suffering from Kidney Disease due to impure drinking water. In Anuradhapura and the surrounding areas, the water is simply too impure. Most people suffer from the same ailment. No matter how much they boil the water or even strain it, the impurities in the water probably cause too much damage. And as we all know, kidney disease is not anything they can recover from or even receive medication easily for.
