This is what a normal pyloric sphincter looks like. The pylorus is located at the bottom of the stomach, at the opening of the small intestine. This is what keeps food in your stomach until it reaches a certain digestive point before moving on to the small bowel to have the nutrients absorbed. Yesterday we discovered that Allies pylorus was completely sealed shut. That bad boy was on lock down. The doctor ran into resistance trying to insert the scope but was able to get through, even then the pylorus only opened up enough to accommodate the scope. He then took a balloon and opened up/ dilated the pylorus. We are hoping this is one of the issues going on and that she will soon have some relief. This will not solve the mystery of her small bowel or colon but it could give her stomach a break. Now we wait for the symptoms to return. This will tell us how long the dilation last. If it last for a minimum of 3 months we will do another scope and dilate again. Anything under 3 months and we will evaluate if a more permanent surgical solution would be best. This is very odd that Allie is having this problem. It is almost identical to Pyloric Stenosis that is very common in infants, most commonly males between 2-6 weeks of age. Now we wait. We see how she tolerates feeds. Last night she did well with pedialyte. She took in 480 mls and didnt vomit but just a small amount until this morning. All in all, she didnt tolerate about 100 ccs of that. In other words she took in about 16 oz and didnt tolerate just over 3 oz. THIS IS HUGE!! Later today we will try again and soon we will reintroduce formula for the first time in 6 months. If she does well, basically if she tolerates any of the formula for any amount of time, even just an hour we will be able to reduce her TPN and come down off her iv some!!!
