Thoughts? 71 year old male, found down in his front yard underneath a running riding lawnmower (about 12 inches of snow on the ground, ambient air temp around 15F). First responding BLS crew moved the patient indoors to his home, removed wet clothing, and began both passive rewarming and high-flow oxygen therapy for presumed CO poisoning. The patients caregiver reports a PMHx of hyperlipidemia, HTN, diabetes, and prior MI. No known allergies, meds include ASA, glyburide, metformin, omeprazole, Prilosec, Prinivil, Ultram, atorvastatin. At the time of the tracing, the patient is shivering moderately, is extremely lethargic, and specifically denies chest discomfort or any angina equivalents. HR 94/min (rhythm strip has frequent unifocal PVCs), RR 22/min, BP 142/86, SpO2 not reading due to low perfusion. SpCO monitoring isnt available.
