Three BWL commissioners told the State Journal in recent days that utility executives never informed the board about the workplace safety violations or nearly $14,000 in fines. BWL settled with the Michigan Occupational Safety and Health Administration in December 2011 and paid fines in February 2012. Commissioners knew the July 26, 2011 chemical accident had ravaged the plant and forced evacuation of a neighborhood, but when they asked where things stood as recently as last month, BWL Water Director Dick Peffley and General Manager J.
