Time for Dr. Chan’s Media Review! Rating: 2 out of 4 Stethoscopes Why? This Scrubs clip shows how physicians should NOT document about their patients. Physicians would never document that a patient is “moderately attractive” in the medical record. Such a comment would be considered quite unprofessional and would add no medical knowledge towards any type of diagnosis or treatment. The medical chart exists to record what transpires in the hospital as well as to communicate with other health care professionals. A more standard and appropriate way to describe what JD sees is: “The patient appears her stated age with good hygiene, lying in bed, and in no apparent distress.” How accurate? Not very. Tape recorders are no longer in use in most hospitals. There are dictation systems that are accessed through the internal phone system and the transcribed reports then appear in the electronic medical record a few hours later. Best Hollywood Moment? Turk sliding across the nurses’ station counter. This is very hard to do and slightly dangerous. I tried it once in my residency program and nearly fell down.
