Maintain and manage the teams. Manages the clinical and operational aspects of the community based health services and working as part of a dynamic interdisciplinary team Planning, coordinating, implementing and evaluation of ward Based Outreach Teams in the City of Tshwane (COT) using the Community Orientated Primary Health Care (COPC) model to implement Ward Based Outreach Teams.
