Hello fellow Veterans, My name is Alfredo Massa Reyes and I am a PhD candidate at the University of Central Florida and a former member of the U.S. Navy. In an effort to complete my degree requirements, I am undertaking a research projects whose goals are to explore the effects of access and need for care factors on health services utilization by the returning veterans of Operation Iraqi Freedom and Operation Enduring Freedom that have been exposed to warm time situations that could lead to post traumatic stress syndrome. As you know, many of our returning veterans (both male and female) were exposed to scenarios that ranged from deadly to both physically and mentally incapacitating. Many veterans have sought support and treatment; however, it is possible that not all who should seek support and treatment post-deployment have in fact done so. Additionally, for those who have sought support and treatment, I want to find out how satisfied they were with the care they received. Towards that end I have crafted an online survey to help me gather the necessary information to make an informed analysis. The results of this survey will assist in making correlations between health care service utilization / satisfaction and characteristics of returning veterans. The survey will provide useful information as regards actual usage and impact of health care provided to our returning veterans. I am asking you, a veteran from Iraq or Afghanistan, to participate in an anonymous survey consisting of 70 multiple choice questions. As a survey participant: 1. You participate of your own free will. 2. It is recommended that you undertake the survey at a private location of your choosing where you feel adequate privacy is available. 3. You can stop at any time and terminate the survey if you do not feel comfortable answering any of the questions. 4. You understand that due to the anonymous nature of your participation, survey results cannot be made available to you. 5. If you agree to participate in the survey, none of your responses can be traced back to you and personally identifiable information will NOT be asked of you (no name, no ID, no address, no phone number--nothing) 6. The survey will require 20-45 minutes of your time depending on your internet connection. 7. This survey is in no way affiliated to the VA or other veterans’ organization nor the U.S. government.
