What is ResolutionCare? {Community-Built, Home-Centered, Palliative Care team using house calls and telemedicine connections to do more care, as well as raise the bar wherever people receive care for serious illness. Palliative Care means matching goals to treatment....better symptom control and guidance.} ResolutionCare is building a team to care for people facing serious illness in the comfort of their homes. We also share knowledge and nurture competence in healthcare providers that work in the many settings where people with serious illness receive medical care. We passionately support the highest quality of life and empowered choice making with the understanding that people are much more than their bodies. The ResolutionCare team will dedicate their skills, experience, and wisdom to managing symptoms and supporting people in making the best possible choices for themselves on their own terms. Beginning with a Palliative Care physician and a Coordinator, the full team will grow to include a Nurse Practitioner, a Social Worker, a Chaplain, Community Health Worker, and Support Staff as the necessary resources become available. First, the ResolutionCare Team restores tradition and returns the center of care for people with serious illness to where it belongs... to the home. Through house calls and virtual house calls (using videoconferencing technology), we expect to dramatically increase our capacity to meet the growing demand for Palliative Care in our community, and to offer consultation remotely where specialty Palliative Care services are lacking. Secondly, the ResolutionCare team, in partnership with Project ECHO at the University of New Mexico, will use the same videoconferencing technology to share knowledge and insight to care providers in Primary Care, Oncology practice settings, as well as anywhere else people with serious illness receive medical services, including Nursing Homes and Assisted Living Facilities. ResolutionCare’s mission is to bring capable and compassionate care for everyone, everywhere as they approach the end of their lives. Why is Now the Right Time for ResolutionCare? The quality of care for individuals facing the end of their lives—and the lives of their loved ones—is often characterized by unmet needs for even basic symptom control, confusion in the navigation of many healthcare delivery systems, and the loss of empowerment and self-determination. The field of Palliative Care has gotten enormous traction with policy makers and professional organizations over the last 10-15 years and is currently entering back into public conversation. In the face of the shear demographic weight of the aging ‘Baby Boom’ generation, the demand for Palliative Care is growing exponentially while the supply of skilled and inspired professionals lags far behind. The result is unnecessary suffering and costs, and an enormous opportunity to make a meaningful difference for everyone at the most vulnerable moment imaginable. Professional organizations such as the American Society of Clinical Oncology and the Institute of Medicine have issued strong statements declaring the “current cancer care delivery system in crisis” due to the absence of basic symptom control, ineffective communication, and the consequences of patients unengaged in choice making. The 2013 Institute of Medicine white paper calls for the upstream participation of Specialty Palliative Care collaboration in the interdisciplinary care of patients with incurable cancer and /or heavy symptom burdens. Similarly, the more recent Institute of Medicine report from September 2014 on Dying in America, further calls for more Specialty Palliative Care as well as more effective Primary Palliative Care to be practiced within all settings where people with serious illness seek care. ResolutionCare finds opportunity in the mismatch between the demand for high quality Palliative Care expertise and its limited availability. By supporting existing community-based resources and models of care, ResolutionCare increases capacity through a “hub and spoke” learning model. We are the hub, health care providers and patients are the spokes. How Did Our Story Begin? In Humboldt County alone, Dr. Fratkin has attended to about more than 1100 deaths in the last 6 years alone with an average of 4-5 family members locally that have directly experienced the value of this work. The team of professionals that this crowdfunding campaign exists to fund adds greatly to ResolutionCare’s ability to make palliative care a priority in our health care system. How Does ResolutionCare Work? ResolutionCare is a social enterprise: an organization that contains both a mission oriented 501(c)3 Non-Profit Corporation and financially self-sustaining Professional Corporation at the same time under the same umbrella. This type of organization is the crossroad of social impact and financial sense leaving flexibility for developing diversified revenue streams. Once fully funded, ResolutionCare will comprise of an amazing team of top shelf professionals: a physician, nurse practitioner, social worker, chaplain, community health worker, and support staff. The combined experience, expertise, and commitment to share what we know guarantees high impact palliative care service delivery. Through an affiliation with Project Echo at the University of New Mexico, we will use the ECHO model to link our extraordinary Palliative Care team with high impact physician groups (Primary Care, Oncology, Nephrology/Dialysis) and other care-giving professionals to become part of a learning community. Through case-based learning, providers receive: Mentoring and Feedback Continuing Professional Education from an interdisciplinary team that knows how to deliver high quality palliative care Patient Satisfaction Cost savings to people that experience easily avoidable suffering Through house calls and virtual house calls, patients receive: · Care in their homes long before hospice can help · Autonomy, empowerment and self-determination as they realize that they are included in choice making · Incalculable savings in time, money and wellbeing by avoiding hospitalizations, unnecessary costs and stress How Does the World Stand to Benefit? ResolutionCare is a global asset. Telemedicine allows us to extend training and consultation to any end of life care facility worldwide. We are gaining positive traction with international health care providers in Africa, Asia, Canada, and Europe. As we move forward with our mission, we trust that we will become more and more international. Start-up capital is needed to invest in a lean and extraordinary interdisciplinary professional team that will: Provide services Develop partnerships Evolve working administrative structures that support the mission ResolutionCare is seeking $100,000 to deliver high-powered, dynamic Palliative Care services both locally in Humboldt County, California and nationally via cutting-edge web-based telemedicine. This funding goal will enable us to achieve our minimum viable fulfillment of initial staffing and technology infrastructure. As a Stretch Goal, $140,000 will allow us to bring on an administrative assistant that will further amplify our efforts to provide excellent services to healthcare providers and patients. The further you take us, the closer we will get to a fully staffed team including Nurse Practitioner, Chaplain, Social Worker, Community Health Worker, and Operational Manager! Dr. Michael Fratkin, founder and Palliative Care specialist at ResolutionCare, will be simultaneously funding operating costs via revenue derived from fees for service and a hospital directorship position to ensure the sustainability of the delivery of top-notch Palliative Care and to maximize the impact of the funds raised via this campaign. This campaign constitutes only a portion of our overall long-term strategic budget. Our stripped down funding goal represents the minimum that we need to begin implementing our plan to change the Palliative Care industry for the better.
