What medication is most likely responsible for these laboratory abnormalities? You are asked to see a 15-year-old girl for menorrhagia. She had onset of menarche at 12 years of age, with irregular menstrual cycles lasting 3 to 5 days. Recently, her menses have been lasting 7 to 10 days, with heavy bleeding for the first 5 days. She has not had any other bleeding problems, but she reports frequent migraine headaches for the past year. There is no family history of bleeding problems. You perform a complete hemostatic workup. The only abnormalities identified are a prolonged closure time with epinephrine on the platelet function analyzer and a significantly decreased platelet aggregation response to arachidonic acid. Platelet aggregation is normal to all other agonists, and her prothrombin time and activated partial thromboplastin time are also normal.
