bronchogenic_ca. The role of HIV in bronchogenic carcinoma is controversial.( Increased risk from 6-15 % to no increaed risk). The increased incidence might be due to increased exposure to other risk factors.These occur in a more younger population, more common in males,more advanced with reduced survival time and usually adenocarcinoma or poorly differentiated. Diagnosis can occasionally be delayed due to non specific featuresThe doubling time is rapid. The imaging appearances are the same as non HIV patients.It can present as a central or peripheral mass, cavitation, consolidation/atelectasis, mediastinal lymphadenopathy, bony destruction, pleural effusion or mass. In 90 % , it occurs in the upper lobe and is more commonly peripheral.The etiology is believed to be immunosupression( although there is no direct correlation between CD4 level and carcinoma) or scar carcinoma( as noted by increased incidence of peripheral upper lobe tumours. Patient presented with focal neurological symptoms and confusion. CT scan of brain shows a ring enhancing lesion. The possibilities of glioma, lymphoma and toxoplasmosis were considered. Chest CT shows a large mass, which was proved to be bronchogenic and the brain lesion is metastaic.
