Clinical History CHRONIC LBP; BLE PAIN/NUMBNESS/WEAKNESS; PAIN XS YRS HAS GOTTEN WORSE LAST 2 YRS; NO TRAUMA Technique Multiplanar multisequence MR imaging of the lumbar spine was performed without the administration of IV contrast. Findings There are extensive degenerative marrow endplate changes at L2-L3 and L4-L5. There is grade 1 retrolisthesis of L2 on L3. There is also trace retrolisthesis of L1 on L2. There is disc desiccation at essentially every lumbar level. There is severe disc space loss at L2-3 and L4-5. Moderate disc space loss is also identified at L1-2. Retroperitoneal structures are grossly unremarkable. Paraspinal musculature is symmetric. Vertebral body height is maintained. Evaluation of the individual lumbar levels reveals the following: L1-2: There is a disc bulge with facet joint arthropathy and ligamentous hypertrophy contributing to bilateral lateral recess narrowing. No high-grade central canal or neural foraminal stenosis is appreciated. L2-3: There is a moderate disc bulge with facet joint arthropathy and ligamentous hypertrophy resulting and at least moderate bilateral neural foraminal stenosis. There is also moderate to severe central canal stenosis. L3-4: There is a small disc bulge with facet joint arthropathy and ligamentous hypertrophy contributing to bilateral lateral recess narrowing, severe left neural foraminal stenosis and moderate right neural foraminal stenosis. L4-5: There is a central disc protrusion, facet joint arthropathy and ligamentous hypertrophy contributing to severe left greater than right bilateral neural foraminal stenosis L5-S1: There is a disc bulge, facet joint arthropathy and ligamentous hypertrophy with mild left neural foraminal stenosis. No significant canal or right neural foraminal stenosis is appreciated. Impression Multilevel multifactorial but predominantly discogenic degenerative change with varying degrees of canal and neural foraminal stenosis described in detail above. There is severe neural foraminal stenosis is identified at L3-4 and L4-5 with severe central canal stenosis identified at L2-3.
