yesterday my last client came in at 5.15pm so i was busy until about just after 6pm. Althhoug this last one is a very compliant individual she had to see me for something totally different. worthwhile mentioning.. another default case earlier in the day ... this lady, very big and looking strong on the outside had visited us about a year and a half with ad CD4 of 9! after all the adherence counseling she and her 7 year old son start ART. in the beginning all is going well until missing a review date here and there sets in... so naturally adherence counseling is intensified... however the negative trend keeps on and no matter the fear i put into her she still would miss here and there... then again for a while we do not see her so naturally we follow up and then the nurses tell me that she has recently been admitted at another hospital.. thankfully the good counselor there is able to find out that she is already under care with us so she gets in touch and sends her back... it is unbelievable... stilll quite heavy, she has lost about 10kg in a short period, she has severe oral thrash, is complaining of severe diarrhea which sent her to the hospital and she looks awful... this time, i do not leave it for the counselors alone, i get in there myself and i give her the best counseling i may ever have given to someone in such a condition... being heavy and looking good on the outside does not mean you are in control if you do not adhere to treatment...something bad is just waiting for an opportunity to take over and that is exactly what happened to her... she is pleading and crying and begging but i refuse to be nice this time.. she needs to know she has severely put herself in danger... after hours of talking, i feel she has had enough ... she has been given the needed medicines and we will be keeping a close eye on her...the education, also for those on ARV still needs a lot of attention.....Dr. Naa
