BASA BASA DIN PAG MAY TIME Procedure for Enrolment (Old Student) STEP 1: Secure and Fill-up Preliminary Enrolment Form from your respective college. (Please bring your last semesters blue card/registration card) Submit duly accomplished Student Clearance Form. (Bring all necessary documents such as School ID and Library card) Assessment will be done for your academic status. (Grades will be based from ROG.) STEP 2: Secure Medical Clearance from the University Clinic. (Bring P200 for fee) STEP 3: Proceed to the University Registrar to submit Pre Enrolment Form and secure Registration Card and accomplish. (Bring towel, bottled water and fan to avoid heat stroke) STEP 4: Proceed to the following: 1. Library - Revalidate library card; 2. Secure Insurance at the designated place. 3. Office of Student Affairs - Revalidate Scholars Contract. STEP 5: Proceed to the Registrars Office submit the accomplished Registration Card. Retrieve the students, deans and OSAs copies and submit to respective offices. Secure class cards - YOU ARE NOW OFFICIALLY ENROLLED. STEP 6: Have your Identification Card revalidated at the designated place. STEP 7: Attend the General Re-orientation scheduled by the OSA and respective colleges. STEP 8: Attend the 1st day of classes and submit class card to the respective Instructors. ---------- GOODLUCK! ----------
