Chief complaint: on the brink of mental insanity x 6 hours History of present illness: 27 year old distressed female presents with emotional and psychological turmoil for the past six hours due to chaotic household events including wild and muddy dogs, an overflowing sewer, inability to complete weekly homework assignments, and an overwhelming amount of unfinished housework. Symptoms are accompanied by sleep deprivation, heart palpitations, anxiety, and frustration. Patient denies assistance from her husband, as he is currently in Canada. Assessment: pt requires immediate and intense stress therapy including but not limited to massage, manicure, pedicure, retail therapy in the form of shopping, and a long vacation alone. Plan: lock dogs up in their crates, turn the lights off to ignore the messy and wet house, order a pizza, and pour a glass of wine. Repeat as frequently as needed. Follow up in the am. Well at least I am learning something in grad school.....
