Good Evening, I hope this finds you and your pets enjoying the summer months. We have some exciting news! Home Veterinary Care has entered into an agreement to merge with Laurelton Animal Hospital on August 1, 2013. This decision represents an exciting new chapter for our practice and for you, our clients. As always, our unwavering focus is on delivering quality patient care and a positive client experience in order to maintain a strong human-animal bond. Home Veterinary Care appreciates the trust and confidence you’ve placed in us over the past 21 years and we are certain that you will find Laurelton worthy of the same. We recognize that you have many choices for your pets’ care and our continued commitment to you is to keep earning that trust every day. Laurelton Animal Hospital, located on Empire Boulevard in Irondequoit, is staffed with two veterinarians (Dr. Kenneth Boehm, hospital owner, and Dr. Jessica Demers), two licensed veterinary technicians, and four support staff. At Laurelton Animal Hospital, my position will be Associate Veterinarian. In addition to compassionate patient care, Laurelton provides on-site surgical and dental procedures, and has digital x-ray capabilities, as well as offering telemedicine. Laurelton offers evening and Saturday hours for your convenience. Pets who are due for vaccines and annual physical exams, or who need medical attention through the month of July will be seen by Home Veterinary Care. Starting in August, reminder cards will be sent with Laurelton contact information on them. If you wish to call Laurelton before August to schedule your pet’s annual exam, you may call 654-6690. Questions and comments you may have: What will happen to my medical records? As of August 1, all medical records will be transferred with me to Laurelton Animal Hospital. But Irondequoit seems so far away! It may seem so, but Laurelton is right off Rte 590. Whether your pet needs his or her annual wellness exam (once a year!) or has an emergency situation, you will receive excellent care at Laurelton. Will I always see you if I make an appointment at Laurelton? This will depend upon the nature of your visit and patient scheduling. If I am not available, an appointment can be made with Dr. Boehm or Dr. Demers, who will take great care of your pet. Will you still offer housecalls? Yes, on a case-by-case basis. What if I have been ordering my pets’ food and medications through MyVetDirect? You will have the opportunity to order through MVD through September 30. My advice is to order several months of food and medication so that your pet’s home-health needs are met until his/her next wellness appointment. After September 30, we will inform you of your ordering options. What if I choose another veterinary hospital? If you would like copies of your records faxed or mailed elsewhere, please contact Katie at hvckatie@gmail (New York State law dictates that we must maintain the original records). E-mail is the preferred method of communication to Katie as we need a written request from the client. After calling the new hospital to determine if they are taking new patients, please provide us with a fax number or clinic address. There will be a $5.00 fee for the transfer of each pet’s records. What if I have questions? As always, you may call the Home Veterinary Care office at 538-2230 (through August 15). Alternatively, you may e-mail Katie at hvckatie@gmail (through September 30). If your question is more appropriate for the Laurelton Animal Hospital Office Manager, call Kelly at 654-6690. Home Veterinary Care and Laurelton Animal Hospital are confident we are making the right decisions to position our clients and their pets for a long-term relationship together. Both practices take pride in our small veterinary practice philosophy of developing a personal relationship with our clients and patients. I want to thank my staff for 21 years of dedication to maintaining the health of your pets. I could not have done this without the assistance of Patti and Katie, and the animal caretakers who have helped throughout the years. With warm regards, Cheryl
