Having read the report into the review into the care of Niall, which details 33 pages, we are now in a position to comment. We welcome the following recommendations and hope they will be adopted and put into practise without delay. We are not in a position to comment on all 33 pages but just to say that there are a number of contradictions, and we feel that there are a number of things that need clarifying further ahead, but we do firmly believe that had these recommendations BEEN in place Niall would be alive today. We would like to urge the HSE (Health Service Executive) to build further on these recommendations, because we believe that there is more that could be put in place. 12.0 Summary of Recommendations Number Recommendation Rating Completion Actions 1 The recommendation from this review is that the Regional Orthopaedic Governance Group review the current patient information sheets that are provided to patients, determine if the information provided is clear, comprehensive and understandable to the patient and if not then the information should be updated to ensure that it is presented in a format that is clear, comprehensive and understandable to patients. The chair of the Regional Orthopaedic Governance Group should nominate a subgroup to action this recommendation within a specific timeframe of approximately 3 months. Ms. P S. and Ms A.S. 2 In relation to the obtaining of consent for a procedure it is essential that healthcare professionals document clearly both the patients’ agreement to the procedure and the discussions that led up to that agreement including the risks and benefits of the proposed procedure. This information should be documented in the patient’s healthcare record. The orthopaedic information leaflets used in the hospital should be reviewed and consideration should be given to making leaflets procedure specific with risks and benefits of surgery documented. Risk and benefits associated with any procedure should To be sent to General Manager, WRH & KOH. Ms P.S and Ms A.S. 25 be provided to patients at their OPD visit where the option for surgery is discussed. This would allow the patient time to consider all the relevant information. The patient should also sign that they have received the relevant information. Time should be incorporated into the patients outpatient visit to allow discussion of the relevant information and to have any of their concerns acknowledged and answered. Documentation of the consent process can be carried out either through the use of a consent form or through documenting in the patient’s healthcare record that they have given consent for their procedure. This would comply with recommended best practice as outlined in the Theme 1 Person Centered Care and Support of the National Standards for Safer Better Healthcare June 2011. 3 The information leaflet given to Mr. NC post procedure needs to be updated to include all complications of the procedure even those which are considered minimal. Consideration should be given to using other methods of communicating relevant information to patients such as phone applications which maybe more user friendly for the younger population of patients. Ms. P.S. and Ms. A.S. 4 In the future following serious adverse events, personnel from the Employee Assistance Programme must be invited to attend relevant hospital and meet with staff in order to facilitate debriefing exercise.
