Iliotibial band (ITB) syndrome involves friction between the ITB and lateral aspect of the knee around the insertion of the band. Common complaints include an ache over the lateral side of the knee during running which is caused by friction or impingement. Features include tenderness over the lateral knee, ITB tightness, weak hip abductors and the presence of trigger points along the ITB. Biomechanical issues of the foot, knee and hip may also contribute to excessive strain on the ITB. Treatment includes symptomatic relief with ice, analgesics and ultrasound as well as activity modification such as avoiding downhill running. Soft tissue therapy aimed at correcting tightness of the ITB, quadriceps and hip rotators as well as gluteal trigger point therapy can help. Frequent stretching of the ITB and other tight muscles are also recommended. A strengthening program may also be put in place if the hip abductors test weak.
