However, in the older age group, it can be difficult to start women on HRT without side effects. *Prevention of cardiovascular disease Although previous observational studies had suggested a protective effect against heart disease, the results of the largest prospective controlled study have in fact shown an increase in the incidence of heart attacks on combined HRT. Until further studies are done, HRT is no longer indicated for the sole purpose of prevention of cardiovascular disease. **Contraindications are as follows. HRT should not be given to women with an estrogen-dependent malignant tumor. Postmenopausal bleeding or abnormal perimenopausal bleeding should be investigated before starting HRT. A history of recent or active thrombosis contraindicates HRT.
