Okay FB peeps (medical or not) I need to VENT. Adam is 36 hours post-op lumbar laminectomy, fusion with rods and pins. He is at a world renowned academic center (Barrows Neurological Center). This morning he started with intractable vomiting and increased neck and back pain. He has been given 7+ medications to relieve the pain, muscle spasms and nausea. The nursing staff have consulted the neurosurgeon, neurosurgery resident and hospitalist. Each of them have ordered different medications. The nursing staff have biasses of what meds they like to use. All meds are ordered as needed. Adam in his snowedcondition isnt able to voice his level of pain. What has happened to everyone working as a team? Where is the pain management team? The growling NP wife demanded that they consult the pain team. No one has documented his intake or output. He has not eaten or urinated for over 12 hours. I asked for a bladder scan and he had over 800 cc in his bladder. When it came time to put a catheter in his bladder I had to demand that they use lidocaine prior to insertion. My heart goes out to the poor patients that do not have a medically competent medical person overseeing their care. The good news is that Adam has a pain management plan and he is now resting comfortably. I want him home ASAP. At this point I could better manage his care physically and emotionally. 😡😥
