PROCEDURES AND REQUIREMENTS IN APPLYING FOR THE AUTHORITY TO OPERATE BINGO BOUTIQUE/TRADITIONAL BINGO TAKE NOTE: STEP NUMBER 3 LETTERS F AND G. (WHY NAG TAPAD MAN LAGI NA DIHA ATBANG CITY HALL - UNSAY TAWAG NINYO ANA? STEP 1 The applicant shall submit an accomplished Letter of Intent to operate bingo at his chosen venue addressed to: MR. CRISTINO L. NAGUIAT, JR. Chairman and Chief Executive Officer Philippine Amusement and Gaming Corporation PAGCOR Corporate Office Hyatt Hotel and Casino Manila M. H. del Pilar corner Pedro Gil Streets Malate, Manila ATTN: MR. EDUARDO D. LAGMAN Assistant Vice President Bingo Department He shall also be required to pay to PAGCOR an application fee of P2,500.00 upon submission of application. STEP 2 An initial screening of the proposed bingo project and ocular inspection of the bingo venue shall be undertaken by PAGCOR-Bingo Department (BD). A proponent whose application passed PAGCORs initial evaluation will be advised by BD to come for an interview and will be given an application packet to serve as guide on the documentary requirements and application procedures. Applicant shall assist the representative/s of BD during the conduct of the ocular inspection and evaluation of the viability of the proposed site. STEP 3 A qualified applicant shall submit the following pre-approval documentary requirements: a) Resolution/Certification/Letter of No Objection (RONO/CONO/LONO) to the proposed bingo operation issued by the concerned City/Municipal Council. If said requirement is not yet available at the time of application, applicant may submit a copy of the application of the RONO/CONO/LONO duly received the Council along with a Letter of No Objection or Endorsement issued by the Mayor. b) Description of Proposed Site c) Vicinity Map (to indicate landmarks) d) Pictures of Proposed Bingo Site and the Surrounding Areas e) Detailed Floor Plan (with measurements) of the Proposed Bingo Boutique to indicate the floor area, PAGCOR Office Space/Cashiers Booth and arrangement of Electronic Bingo Terminals. For traditional bingo operation, the floor plan shall include the location of offices (including the PAGCOR office space), card stockroom, restroom/s, and arrangement of bingo machine, bingo stage, bingo tables and chairs and other bingo hall facilities. f) Distance of Proposed Site from Schools and Churches or places of worship (Note: Proposed Site must be more than 200 lineal meters from schools, churches or places of worship.) g) Distance of the Proposed Site from Existing Operational Bingo Venue/s (Note: Proposed Site must be at least 1,000 meters (1 km.) (via access roads) from existing bingo venue/s and at least 1,000 lineal meters from other gaming establishments approved by PAGCOR h) Feasibility Study on the Proposed Bingo Project i) Architects Perspective (with designs and plans) of the interior and exterior of the bingo venue (to be approved BD prior to construction of bingo hall) j) Lease Proposal/Space Reservation from the proposed sites owner/management or Proof of Ownership if the proposed bingo venue is owned by the applicant k) Copy of Proposal from PAGCOR-approved Electronic Bingo Supplier (for bingo boutique/electronic bingo operation) l) Profile of Commercial Bingo Applicant: (Corporation, Partnership or Sole Proprietorship) m) For Corporation – SEC Certificate of Registration, Articles of Incorporation and By-Laws (Authorized Capital Stock: Php5M for traditional or e-bingo only; Php16M for traditional and e-bingo) (Primary Purpose: Conduct and operation of bingo games) For Partnership – SEC Certificate of Registration, Articles of Partnership (Authorized Capital Stock: Php5M for traditional or e-bingo only; Php16M for traditional and e-bingo) (Primary Purpose: Conduct and operation of bingo games) For Sole Proprietorship – DTI Registration Papers (Capitalization: Minimum of Php3MPhp for traditional or e-bingo; Php5M for traditional and e-bingo) (Line of Business: Conduct and operation of bingo games) n) Latest Income Tax Return of the Corporation/Partnership/Proprietor (stamped “Received” by the BIR or its duly accredited and authorized institutions) o) Bank Statement of Corporation/Partnership/Proprietor with a minimum maintaining balance of Php3M for bingo boutique and Php5M for traditional and e-bingo p) Authority to inspect bank account of Corporation/Partnership/Proprietor q) Duly accomplished, signed and notarized Personal History Statement of each incorporator/each partner/proprietor r) Duly accomplished, signed and notarized Sworn Statement of Assets and Liabilities of each incorporator/each partner/proprietor s) NBI Clearance, Court Clearance and Police Clearance of each incorporator/each partner/proprietor t) Organizational Chart/Structure of the proposed bingo operation u) Accomplished Authorization Form for PAGCOR’S Technical Inspection Team to Conduct Site Inspection STEP 4 Upon the proponent’s full compliance with the abovecited requirements and prior to BD’s recommendation to the Board of Directors of the approval of an application, the proponent shall pay to BD a processing fee of P30,000.00 to cover PAGCOR’s expenses for the facilitation of said application. STEP 5 BD will recommend the approval of a qualified application to PAGCOR’s Board of Directors. A qualified application may be approved within 15 days upon proponent’s full compliance with the pre-approval requirements. STEP 6 Once applicant receives advice of approval of their application by PAGCOR’s Board of Directors, Grantee (approved applicant) shall submit the following mandatory pre-operational requirements: a) Mayor’s Business Permit b) Lease Agreement/Contract with Venue Owner or Proof of Ownership if Venue is owned by the Grantee c) PERFORMANCE BOND in the amount of Php500,000.00 in cash d) Copy of Duly Notarized Agreement with PAGCOR-approved Electronic Bingo Supplier e) The following facilities, equipment and fixtures shall be installed in the bingo venue prior to actual operation: e.1 CCTV cameras with video recording system. (In addition, all bingo venues are required to detail their own security personnel at the site 24/7 unless security is provided by the venue itself) e.2 Sufficient air conditioning system e.3 Keybox with lock (for safekeeping of e-bingo keys) e.4 Working area/office complete with tables and chairs for PAGCOR’s Bingo Monitoring Staff e.5 Signages for electronic bingo operations (must not exceed 3.5 feet by 10 feet and must not employ running lights) e.6 Other equipment/facilities that PAGCOR may require to ensure the integrity and security of the bingo operation Grantee has sixty (60) days upon receipt of the Notice of Approval from PAGCOR within which to comply with above mandatory requirements and commence bingo boutique operation at its approved venue. Failure to comply within the prescribed period shall nullify the Board’s approval of Grantee’s application unless it can present/provide acceptable justification for the delay in the compliance with mandatory pre-operational requirements and commencement of bingo operations at the said venue. Moreover, upon full compliance with the aforementioned pre-operational requirements and completion of the bingo venue, Grantee is required to appear at Bingo Department’s (BD’s) office at the 3rd Floor PAGCOR House, 1330 Roxas Boulevard, Ermita, Manila to sign the Authority To Operate (ATO) Bingo Boutique at the approved site to signify Grantee’s conformity with the terms and conditions stated thereon. STEP 7 Grantee shall notify PAGCOR Bingo Department at least two (2) weeks in advance of the target opening date of bingo venue. STEP 8 Representatives of PAGCOR Bingo Dept. shall conduct a final inspection of the venue and its facilities, electronic bingo machines, and other equipment and paraphernalia one (1) week before the target opening date to verify the bingo venue’s readiness for actual operation. STEP 9 PAGCOR Bingo Department shall issue a clearance to the Grantee to commence bingo operations at its approved venue only upon full compliance with the mandatory pre-operational requirements referred to in Step 6 above and issuance of the duly signed ATO.
